Step 1: Log In to the FORMS website
Log in at https://forms.nycwff.org/wp-admin/.
Step 2: Add New Page
Hover over Pages from the menu on the left, then select Add New.
Step 3: Title
Add the form title
Step 4: Formstack Embed Code
- Paste in the Formstack embed code in the text editor. Make sure you have the “Text” editor version selected (see arrow in screenshot below).
- Note: We recommend using the embed code rather than the HTML, because the embed code will automatically pull in updates to the form.
Step 5: Publish
Click the blue Publish button to add the form to the Forms website.
Step 6: View the Form
You can view the form on the frontend by clicking on the permalink.
